The Foundation Patient & Clinical Pharmacy contributes to the promotion of clinical pharmacy in science as well as the transfer of clinical research findings into practice. Our goal is to improve drug safety and patient safety for each individual patient.

Current Projects

Emergency Department Pharmacist​
Due to a heterogeneous patient group, parallel treatment processes and an acute lack of time, emergency departments are an environment where medication errors occur particularly frequently. In addition, 5-10% of all hospital admissions are due to adverse drug reactions - many are overlooked in the emergency department. The integration of clinical pharmacists can make a contribution to reducing such errors. While the implementation of Emergency Department Pharmacists is already established internationally and shows clear benefits for drug therapy safety, this concept is still in its infancy in Germany. In order to change this, the foundation "Patient & Klinische Pharmazie" is funding three projects that aim to establish the Emergency Department Pharmacist in German emergency departments.

Influence of concomitant medication on side effects of checkpoint inhibitor therapy

As part of a translational research project, the hospital pharmacy and the Department of Medical II of the University Hospital Augsburg are investigating the influence of non-oncological concomitant medication on immune-mediated adverse events (irAE) and the response to therapy with checkpoint inhibitors. Data from cancer patients with solid tumours are analysed retrospectively. Prospectively, a structured drug history is also taken in order to be able to analyse potential influences on the immune profile and therapy response in a more targeted manner with a high level of data quality. The aim is to identify risk factors for irAE, improve treatment decisions and develop strategies for effective side effect management.

Completed Projects

Intensive Care Pharmacist
​
Altered metabolism of drugs, complex treatment regimens and frequent therapy and dose changes in intensive care patients are risk factors for the occurrence of adverse drug reactions in intensive care units. Unlike in Germany, in many international countries the ‘Intensive Care Pharmacist’ is an integral part of the multi-professional team in intensive care units. The foundation Patient & Clinical Pharmacy supported three projects at German hospitals to establish the ‘Intensive Care Pharmacist’ in their facilities.

Renal Pharmacist
​
Between 20-25% of all hospitalised patients have impaired renal function and are therefore at high risk of drug-related problems (DRP). A ‘Renal Pharmacist’ can identify renal DRP through medication reconciliation and thus contribute to increasing the safety of drug therapy. However, this service is not yet standardised in Germany. The foundation Patient & Clinical Pharmacy has funded projects at four German hospitals to establish the ‘Renal Pharmacist’.
