Intensive Care Pharmacist

Altered metabolism of drugs, complex treatment regimens and frequent therapy and dose changes in intensive care patients are risk factors for the occurrence of adverse drug reactions in intensive care units. Unlike in Germany, in many international countries the ‘Intensive Care Pharmacist’ is an integral part of the multi-professional team in intensive care units. The foundation Patient & Clinical Pharmacy supported three projects at German hospitals to establish the ‘Intensive Care Pharmacist’ in their facilities.
Renal Pharmacist

Between 20-25% of all hospitalised patients have impaired renal function and are therefore at high risk of drug-related problems (DRP). A ‘Renal Pharmacist’ can identify renal DRP through medication reconciliation and thus contribute to increasing the safety of drug therapy. However, this service is not yet standardised in Germany. The foundation Patient & Clinical Pharmacy has funded projects at four German hospitals to establish the ‘Renal Pharmacist’.
TDM in PDMS - how to digitize?

As part of the introduction of a new electronic patient data management system (PDMS) in the intensive care unit at the hospital in Heidenheim, existing pharmaceutical services (such as therapeutic drug monitoring, TDM) are also to be digitally mapped. The development and establishment of the electronic PDMS was supported by the foundation Patient & Clinical Pharmacy as part of a project.

Renal Pharmacist Consultant Service
​
Electronic medication modules (computerized physician order entry, CPOE) with decision support systems (clinical decision support system, CDSS) are increasingly being implemented in everyday clinical practice. In a project funded by the foundation Patient & Clinical Pharmacy, the question of whether a ‘Renal Pharmacist’ for patients with renal insufficiency on surgical wards can also contribute to increasing drug therapy safety in the digital patient record setting was addressed.
Drug information for palliative care
(AmInfoPall)
​
The project AmInfoPall offers a supra-regional, free contact point for doctors, pharmacists and nurses on issues relating to palliative drug therapy in German-speaking countries. The foundation Patient & Clinical Pharmacy supported this project for two years (2020-2021).
